Our Services

The Practice Facilitation Team offers a range of support to meet your individual practice needs and will work in partnership with you and your practice to support quality improvement activities you have identified.

Our Practice Facilitators support primary health care practice staff, including:

  • General Practitioners
  • Practice Managers
  • Primary Care Nurses; and
  • Administrative/Reception Staff

Practice Connect Help Desk

Practice Connect and our Practice Facilitation Help Desk are your one-stop-shop for your practice management queries. Our experienced Practice Facilitation Team are available to support you Monday to Friday, 9:00 am to 4:30 pm via the Help Desk. The team may be able to answer your question over the phone or email; suggest resources in our Resource Library; or can arrange a face-to-face visit from one of the team for more extensive support.

Practice Facilitation Visits

When visiting your practice, the Practice Facilitation Team is available to provide tailored support on the following:

  • Accreditation preparation
  • Medicare Benefits Schedule
  • Immunisation initiatives
  • Cancer Screening initiatives
  • Digital Health, My Health Record, ePrescribing, GoShare, Remote Patient Monitoring
  • Set-up of HealthPathways, including login details
  • Data extraction for Quality Improvement, including accessing PHN Exchange for PIPQI reports (login and training)
  • Continuous Quality Improvement – Plan, Do, Study Act cycles
  • Staff training and education
  • Up-to-date information relevant to general practice

  • Quality Improvement activities
  • A suite of other resources to assist your day-to-day operations
