General practices in western Victoria can access a suite of data extraction and analysis tools to improve service planning, reporting and population health monitoring. Western Victoria PHN will transition licences to the POLAR system with Outcome Health.

Using data extraction tools in general practice

WVPHN provides the data extraction tools so general practices can meet data submission requirements for Practice Incentive Payment – Quality Improvement (PIP QI) accreditation.

However, the tools provide a broader range of benefits to general practices looking to improve data quality and ensuring patient records are accurate and up to date.

Practices can also use data extraction tools to help target patients with specific needs of health risk profiles, namely in chronic disease management, cancer screening, immunisations, medication reviews, sexual health, ehealth and preventative health.

To access the tools, general practices must sign the Western Victoria Data Extraction Tool Agreement.
