Advance care planning gives people autonomy and control by planning for their health and personal care into the future should they lose their decision-making capacity. Advance care planning captures people’s values and wishes, allowing them to describe what is important to them and communicating their treatment preferences to other people. It enables them to influence treatment decisions, even when they can no longer actively participate.

GPs can support patients through the planning process, discuss problematic issues and provide information on their health, prognosis and future treatment options. Thereby preparing patients for conscious end of life decision making.
